Latency? Doubling of sound/echo when playing back

Latency? Doubling of sound/echo when playing back

Does anyone know what is going on here? I haven't had this problem until recently. Some times if I reboot the R16, it will go away, but seems to be getting worse, lately. It records fine, it's only during playback.

Just tried recording from my ADAT and it is doing it when recording now, also!

Configuration?

Speakers? Microphones? Microphones picking up speaker output and routing that back to Master Fader?

Any effects on the track outputs or Master input?
